Important Forms and Documents

Please download and complete the following forms prior to attending your course, session or tour.

This is a required document and must be completed before participating in any freediving sessions.

This form is required for all water-based breathwork activities, including cold water immersion, skindiving, snorkelling, and freediving.

*Please refer to Page 1 to determine whether a physician’s evaluation is required for medical clearance.

This form is also required for certified divers participating in freediving training.
