Freediving Practices Statement of Understanding
This PADI Freediver form is a required acceptance document and must be completed to attend freediving sessions.
Diver Medical / Participant Questionnaire
This form is required for all water-based breathwork activities, including cold water immersion, skindiving, snorkelling, and freediving.
*Please refer to Page 1 of the form to determine whether a medical evaluation by your physician is required.
Release of Liability / Assumption of Risk
This form is required for certified divers participating in freedive training
Pool Training Safety Standards
This Form is required for any Pool Training Sessions
