Medical Forms, Documents & Dive Doctors
Important Forms and Documents
Please download and complete these important forms prior to attending your course or training session.
Diver Medical Questionnaire
Your safety in the water starts before you get in it. You’ll need a diving medical from a doctor if:
you answer “Yes” to any question on the questionnaire and are directed to see a doctor, or
you are 45 or older.
If either applies to you, please have your medical signed off before you book, so your spot is secure and there are no surprises on the day.
Getting your medical
Any willing GP can do it. When you make the appointment, ask for a recreational diving medical for freediving.
Not every GP is familiar with diving medicals, so call ahead to check they offer them.
Print the form below and take it with you to your appointment.
Bring your signed form to your course, or email us a copy beforehand.
Not sure where to go? All doctors listed below are known to offer this service in Tasmania.
